RN Case Manager/Medallion

Provides services to clients in accordance with the State Nurse Practice Act, agency policy, and accepted professional standards of practice.  The Registered Nurse Case Manager provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

Responsible for the outcome of applicable OBQI/QA studies.

Reviews all processes on a yearly basis to assure those processes are current and within or exceeding guidelines.

Requirements


* Current licensure as an RN in the state of Montana


* Current CPR certification

* Ability to move independently or with reasonable accommodation within the facility and community.


* Applicant must be physically and mentally able to perform all requirements of the job.


* Must have a reliable vehicle.


* All positions require the completion of a background check.

* Applicant must pass a DMV check.

Experience


* 0-2 years


* Experience in geriatrics care helpful


* Associate's degree


* Graduate of an accredited school of professional nursing
