Treatment Nurse LVN Full-Time
As a Treatment Nurse LVN at Oakwood Gardens Care Center, you will be responsible for providing skilled nursing care to residents in a skilled nursing facility (SNF) setting.
You will focus on wound care, managing skin conditions, and assisting in developing care plans in collaboration with other healthcare professionals.
You will ensure that residents receive the highest standard of care in alignment with facility policies, state regulations, and best nursing practices.
Key Responsibilities:
Wound Care Management: Assess, plan, and implement treatment for residents with wounds, pressure ulcers, and other skin conditions.
Monitor Skin Integrity: Regularly assess residents for changes in skin condition and implement preventative care strategies to avoid skin breakdown.
Documentation: Maintain accurate records of treatment plans, resident progress, and changes in condition using electronic medical records (EMR).
Collaborate with Care Team: Work closely with physicians, Registered Nurses (RNs), CNAs, and other healthcare team members to develop individualized care plans.
Resident & Family Education: Provide education to residents and their families on wound prevention, treatment, and skin care.
Compliance: Ensure all care is delivered in compliance with federal, state, and local regulations, including those specific to skilled nursing facilities.
Medication Administration: Administer medications related to treatment as prescribed, following proper protocols.
Infection Control: Adhere to infection control procedures and assist in the prevention of the spread of infections within the facility.
Emergency Response: Respond to and report any changes in residents' conditions, participating in emergency care when necessary.
Qualifications:
Licensed Vocational Nurse (LVN) in good standing with the California State Board of Nursing.
Experience in wound care or treatment nursing in a skilled nursing or similar setting is highly preferred.
Ability to multitask and handle a fast-paced work environment.
Strong knowledge of wound care protocols, infection control practices, and skin integrity management.
Proficiency in electronic medical records (EMR) systems.
Excellent communication skills, both verbal and written, to effectively interact with residents, families, and healthcare team members.
Physical Requirements:
Ability to stand, walk, and perform physical tasks for extended periods.
Ability to lift, push, or pull up to 50 pounds as necessary.
