Position Summary:

Join PACE! As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will work closely with participants and their caregivers to help them continue to live safely in their homes and communities as independently as possible. You will work in an integrated primary care and behavioral health setting to support participants in their homes and at one of our PACE centers.

Social services case management position in the PACE Social Work Department assisting participants with various "social determinants of health" (SDOH) needs including housing, food insecurity, benefits and more.

Work closely with a supportive interdisciplinary medical team supporting the PACE participants and their families.

Full-time position to act as case manager and support for the Neighborhood PACE Social Work department.

Work with older adult participants and families to assist with vital "SDOH" (Social Determinants of Health) issues including housing, food insecurity, benefits access, and more.

An ideal candidate will be knowledgeable about community resources, be detail oriented, able to multitask, and possess excellent communication and computer skills.

Must hold a Bachelor’s degree in social work or related field OR have two years of relevant work experience in a community setting.

Bilingual (English/Spanish) required.

Automobile required.

Participant Navigator Responsibilities


* Builds trusting relationships with participants



* Provides assistance with housing applications; maintains records of pending applications; provides follow-up on applications


* Acts as liaison and provides housing advocacy with various housing organizations on behalf of participants


* Assists with case management tasks including, but not limited to: Food Stamps/DTA applications and re-certifications, Safelink/Assurance phone assistance, scheduling RIDE interviews, Social Security income verifications, calling Social Security to troubleshoot issu...
