What You'll Do:
? Oversee and coordinate all social services for residents
? Support admissions, discharge planning, and community outreach
? Advocate for resident rights and quality of life
? Lead family meetings and provide emotional support
? Ensure regulatory compliance and accurate documentation
? Collaborate with interdisciplinary teams to support holistic care

What We're Looking For:
