We're currently seeking a compassionate, detail-oriented Social Services Assistant to support our residents and families through care planning and transitional support.

? Position Details:
? Part-Time Schedule
? Starting at $17/hr
? Located in a warm, team-centered environment

Key Responsibilities:
? Assist with resident admissions, discharges, and transfers
? Help develop and update individualized care plans
? Support residents and families through emotional and social challenges
? Maintain accurate documentation and communication with care teams
? Advocate for resident rights and well-being
? Coordinate with community resources and services as needed

Ideal Candidate Qualities:
✔️ Empathetic and patient, especially with memory care residents
✔️ Strong organizational and interpersonal communication skills
✔️ Experience in long-term care or healthcare setting preferred
