We are a skilled nursing facility located in the heart of Marin County, and we are looking for a licensed LVN or RN with a proven track record in MDS coordination.

What We're Looking For:


* Valid California LVN or RN license


* Proven experience as an MDS Coordinator in a skilled nursing facility


* Deep understanding of PDPM, care planning, and state/federal regulations


* Ability to collaborate across interdisciplinary teams with clarity and confidence


* Strong organizational and documentation skills
