Job Summary

Kern River Transitional Care Center, a premier skilled nursing facility in Bakersfield, CA, is seeking a dedicated Admissions Concierge Coordinator to serve as the welcoming face of our facility.

This role is essential in ensuring a smooth and positive admissions experience for residents and their families.

The Admissions Concierge Coordinator will facilitate admissions, provide exceptional customer service, and assist with administrative duties to create a seamless transition into our care community.
Key Responsibilities


* Resident Admissions Coordination
+ Serve as the primary point of contact for new residents and families during the admission process.
+ Guide residents and families through admission paperwork and facility orientation.
+ Work closely with hospital case managers, physicians, and referral sources to coordinate admissions efficiently.


* Customer Service & Hospitality
+ Greet residents and families upon arrival, ensuring a warm and welcoming experience.
+ Provide personalized attention and address concerns or questions during the admission process.
+ Maintain ongoing communication with families to ensure a smooth transition and positive experience.


* Administrative Support
+ Assist in verifying insurance eligibility and benefits.
+ Ensure all admission documentation is completed accurately and in a timely manner.
+ Update records and maintain compliance with state and federal regulations.


* Facility Tours & Marketing Support
+ Conduct facility tours for prospective residents and families, showcasing the center's services and amenities.
+ Represent the facility at community outreach and networking events to build positive relationships with referral sources.


* Collaboration & Communication
+ Work closely with the admissions, nursing, and administrative teams to coordinate smooth resident transitions.
+ Maintain open and professional communication with all departments to support high-quality care and resident satisfaction.

Qualifications


* High school diploma or equivalent required; associate's or bachelor's degree in healthcare administration, business, or a related field is a plus.


* Previous experience in admissions, customer service, or healthcare administration (preferably in a skilled nursing or long-term care setting).


* Strong interpersonal and communication skills with the ability to build positive relationships.


* Proficiency in Microsoft Office and electronic medical record (EMR) systems.


* Ability to work in a fast-paced environment while maintaining attention to detail.
