Sequoia Transitional Care is seeking a skilled and compassionate Registered Nurse (RN) to join our nursing team as a PM Floor Nurse.

This is a full-time, weekday position with some flexibility, ideal for a nurse who thrives in a collaborative, resident-focused environment.

We're offering competitive pay and a $1,500 sign-on bonus for candidates who are ready to make a difference in long-term and transitional care.
?️ Your Responsibilities:


* Provide direct nursing care to residents in accordance with individualized care plans and physician orders


* Administer medications and treatments accurately and on time


* Monitor and report changes in resident health or behavior to appropriate team members


* Coordinate with CNAs and interdisciplinary staff to ensure consistent, quality care


* Maintain accurate documentation using facility EMR (PointClickCare preferred)


* Assist with admissions, discharges, and transitions of care


* Respond to emergencies and follow established protocols


* Uphold infection control and safety policies

Qualifications:


* Current and active RN license (California)


* CPR certification (required)


* Experience in a skilled nursing or long-term care facility preferred


* Strong clinical judgment, communication, and leadership skills


* EMR experience (PointClickCare is a plus)


* Compassionate and resident-focused mindset
