Position Summary:

This role is essential in overseeing a streamlined, patient-centered intake process while driving quality improvement initiatives.

The Clinical Intake & Quality Improvement Project Manager will work closely with the ARCH, prevention and screening, and clinical teams to ensure efficient patient eligibility verification, insurance enrollment, and access to care services.

Additionally, this position will lead and support Continuous Quality Management (CQM) initiatives, leveraging data-driven strategies to enhance patient care, engagement, and health outcomes.

Essential Duties & Responsibilities


* Oversee and optimize the clinical intake process, ensuring a seamless, patient-centered experience by coordinating with ARCH, prevention and screening, and clinical teams to assess eligibility, facilitate insurance enrollment, and streamline service access.


* Execute Continuous Quality Management (CQM) initiatives, working closely with department leadership in designing and implementing performance improvement projects (PIPs) that enhance patient care, treatment engagement, and health outcomes.


* Ensure compliance with Ryan White Program eligibility requirements by managing the collection of income, residence, and HIV status documentation, obtaining consent forms, and overseeing the verification process for newly diagnosed, transferred, and returning patients


* Serve as a key liaison for new patients, working alongside the Linkage & Retention Coordinator to facilitate smooth transitions into care and enhance patient engagement.


* Manage insurance enrollment and verification efforts in collaboration with Patient Services and Medical Case Management (MCM) teams, supporting patients in securing appropriate healthcare coverage.


* Conduct and enhance the annual eligibility review process for Ryan White patients, ensuring timely compliance with program standards and contractual obligations.
