Manager, End Of Life Care - CESD

Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community.

The healthcare teams at St.

Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. 

Job Summary: Under the supervision of the In-Home Services (IHS) Director, the manager of Comfort Care and End of Life services oversees the day-to-day operations of the comfort Care/End of Life Program for St.

Paul’s PACE.

The position provides direct supervision of IHS case managers (RN’s, LVN’s), End of Life Nurses (RNs and LVNs), Home Health Aides (HHA) and department supportive staff.

The position provides supervision visits to IHS staff in the field to evaluate skill and performance.

The position assists the IHS Director in policy formulation and program development and implementation.

This position is responsible for ensuring compliance with annual budgets and financial objectives, company policies and procedures, and guidelines.

The position ensures timely completion of participant assessments to meet any policy and regulations.

The position acts as the liaison between any contracted hospice agency and St.

Paul’s PACE and ensures compliance with staff competency requirements.

The position maintains the participants’ medical record thorough documentation and communicates participants’ changes and progress to appropriate staff.

Duties and Responsibilities:


* Oversees the supervision of the IHS End of Life care staff and ensure
adequate department staffing and staff performance.


* Provide leadership, supervision, and support to clinical staff, fostering a
culture of professional growth, accountability, and teamwork.


*  Ensures staff perform assessments of participants in their home environment to determine their individual needs and ensures coordinated plan of care with care giving resources.


* Ensures coordination with the Interdisciplinary Team to develop
a care plan for each participant and that participants are in their least restrictive environment.


*  Functions as a member of the interdisciplinary team.

Maintains regular attendance and participates at in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for ongoing care delivery.


* Identify opportunities for program development and expansion, including new service offerings and partnerships to enhance the scope and impact of end-of life services.


* Completes orientation, initial and annual competencies, supervises and
evaluates performance of IHS end of life staff.
