Hanford Post Acute, a premier skilled nursing facility serving the Visalia area, is looking for a dedicated and compassionate Physical Therapist Assistant (PTA) to join our rehab team on a per diem basis.

Key Responsibilities:


* Provide physical therapy treatments under the supervision of a licensed Physical Therapist


* Assist residents in improving strength, mobility, and functional abilities


* Monitor and document resident progress toward therapeutic goals


* Communicate effectively with the interdisciplinary team to ensure coordinated care


* Maintain accurate records in compliance with state, federal, and facility requirements


* Educate residents and families on exercises and mobility techniques


* Participate in team meetings and care planning as needed


* Ensure treatment areas are safe, clean, and properly equipped

Qualifications:


* Current California PTA license in good standing


* Graduate of an accredited Physical Therapist Assistant program


* CPR certification (required or obtained upon hire)


* Experience in a SNF or geriatric care setting preferred


* Strong communication, documentation, and patient care skills


* Familiarity with EMR systems (Casamba a plus)
