Your Role:

As a Licensed Practical Nurse (LPN) at Centennial Post Acute, you'll play a vital role in improving the health and well-being of our residents.

You'll work closely with a collaborative healthcare team to deliver compassionate care that makes a real difference.


* Direct Patient Care: Provide high-quality, resident-centered care in accordance with nursing standards and protocols.


* Monitor Resident Progress: Track resident progress, assess needs, and adjust care plans to ensure optimal outcomes.


* Resident and Family Education: Educate residents and their families on health management, treatment options, and wellness strategies to empower them in their care.


* Collaboration: Work with multidisciplinary healthcare teams to coordinate and deliver the best care possible, ensuring that each resident's unique needs are met.

What We're Looking For:

* Current LPN license in Alaska.
