Licensed Vocational Nurse II - Behavioral Health Youth Enhanced Care Management - Mid-County

The Riverside University Health System - Behavioral Health Department is seeking a dedicated Licensed Vocational Nurse (LVN) to join our Youth Enhanced Care Management Program located in the city of Perris, CA.

As a licensed vocational nurse care manager, you will play a vital role in coordinating care for youth with complex medical and behavioral health needs.

The most qualified candidate will have experience or familiarity with behavioral health and substance use disorders.

Have strong knowledge of medical terminology and care coordination for youth services.

Incumbents must have the ability to work independently in a field-based role, traveling to members' homes and other various locations throughout Riverside County.

A valid California driver's license and reliable transportation is required for the position.

Key Responsibilities:


* Serve as a lead care manager, ensuring high-quality care coordination for youth with significant medical and behavioral challenges.


* Conduct health screenings and assessments to identify the needs of program participants.


* Develop and implement care plans tailored to each youth's unique needs.


* Follow up with healthcare providers to ensure members are linked to appropriate services and levels of care.


* Collaborate with a multidisciplinary team to enhance care outcomes and improve the well-being of program participants.


* Travel to members' homes to provide in-person support and care coordination.

9/80, with every other Friday off

https://www.ruhealth.org/behavioral-health• Provide nursing care to patients within the scope of the Nurse Practice Act for vocational nursing; prepare patients for physicians, registered nurse, physician assistants, and nurse practitioners, and care for patients during treatment; administer medications using subcutaneous, intramuscularly, intradermal, subdermal, and/or oral methods.

• Take temperature, pulse, respiration and blood pressure of patients as necessary; apply and change dressings and cares for wounds as necessary; assist physicians, registered nurses, and mid-level providers in performing examinations, tests, respiratory care and other treatments including minor surgical procedures, PAP smears and colposcopies.

• Perform venipuncture, injections of medications including biological immunizations for pediatrics and adults, including the provision of Tuberculin skin tests, Coccidioidin skin tests, and Histoplasmin skin tests in the course of a tuberculosis control program.
