MDS Nurse (Minimum Data Set Nurse)
Job Summary:
The MDS Nurse provides oversight and coordination of the Minimum Data Set (MDS) process, ensuring compliance with regulatory guidelines and optimizing clinical and financial outcomes.
This role includes supervising PDPM (Patient-Driven Payment Model) meetings, tracking performance metrics, providing training, and ensuring accurate and timely assessments for residents.
Key Responsibilities:
PDPM Oversight & Data Monitoring:
* Leads and oversees weekly PDPM meetings for skilled patients.
* Tracks and monitors the facility's effective utilization of PDPM tools, including but not limited to:
+ Physician Diagnosis Verification
+ Admission/Discharge CSR (GG)
+ Weekly CSR
+ PDPM Weekly Tracking Tool
+ PointRight
* Analyzes and monitors Quality Measures (QM) and Quality Assurance and Performance Improvement (QASP) data on a monthly and quarterly basis.
* Provides clinical recommendations based on data analysis to optimize resident care outcomes.
MDS & Assessment Management:
* Conducts and ensures accurate and timely completion of all MDS assessments, including:
+ PPS Medicare
+ Quarterly and annual reviews
+ Significant change assessments
* Maintains an accurate schedule for all MDS assessments and ensures compliance with regulatory timeframes.
* Ensures thorough and complete documentation, adhering to federal, state, and medical standards.
* Verifies and submits electronic MDS data, correcting errors when necessary.
Resident Care Coordination:
* Assesses and determines the health status and level of care for all new admissions.
* Communicates residents' level of care to all disciplines and coordinates interdisciplinary participation in completing MDS assessments.
* Schedules and conducts resident care conferences, ensuring completion of MDS reviews beforehand.
* Assists in developing, reviewing, and revising resident care plans to ensure individualized, goal-oriented care.
* Tracks and manages all level-of-care changes within the facility, notifying relevant departments and processing necessary documentation.
Training & Compliance:
* Provides training on PDPM and MDS processes for new MDS Coordinators.
* Ensures staff adherence to MDS regulatory requirements and best practices.
* Maintains confidentiality of all resident care information, including protected health information (PHI).
Supervisory Responsibilities:
* This position includes supervisory duties, overseeing MDS processes and training new staff.
Qualifications:
Education & Experience:
* Graduate from an accredited School of Nursing; Bachelor of Science in Nursing (BSN) preferred.
* Must be in good standing with the State Board of Nursing and maintain all required continuing education/licensing.
* Minimum 3 years of supervisory experience preferred.
