Responsibilities:


* Evaluate patients within 24-hours or one business day of physician referral; document any delays in the medical record.


* Develop and implement effective treatment plans, obtaining approval from the referring physician.


* Administer treatments according to the physician's treatment plan.


* Train and support the nursing department in the use of Restorative Aides.


* Supervise Physical Therapy Assistants in patient care and related activities in compliance with state practice regulations.


* Communicate patient progress, issues, and plans with supervisors and other healthcare team members.


* Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences, as needed.


* Conduct in-service training programs for facility staff.


* Document evaluations, daily treatment notes, weekly progress notes, recertifications, and treatment changes as per state practice acts and payer requirements.


* Educate patients' families and nursing staff on maintenance programs and caregiver training for discharge preparation.
