Spiritual Counselor
San Diego’s St.
Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community.
The healthcare teams at St.
Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a Full Time position.
The typical schedule is Monday - Friday, 8:00AM - 4:30PM.
Job Summary: The Staff Spiritual Counselor is responsible for the delivery of Spiritual care to Participants, their loved ones, guests, and, as appropriate, staff.
Provides Spiritual care services in such a way as to accomplish the objectives of St.
Paul’s PACE.
Functions in such a manner that reflects the mission and vision of St.
Paul’s PACE.
Duties and Responsibilities:
* Participates in delivering and arranging for appropriate Spiritual care to Participants, their loved ones and staff in specific areas of St.
Paul’s PACE.
* Actively participates in the delivery of Spiritual care, responds to requests for Spiritual care in a timely manner, participates in the offering of worship opportunities, and participates in the meeting of spiritual and sacramental needs.
* Willingness to work with and meet the spiritual needs of people of all religious faith, age, gender, sexual orientation, race, and ethnic backgrounds
* Participates in regular and special religious services as required.
* Assists in the coordination of clergy and Spiritual visitors from the religious community.
* Maintains liaison between local clergy and Participants.
* Assesses Participants and family spiritual needs and assists families in using their faith in dealing with illness, trauma, and stress; visits newly admitted Participants in all St.
Paul’s PACE facilities; visits Participants in acute hospitals; and follows up with Participants from previous visits when appropriate.
* Documents Participants and family service provided.
* Assists Participants and families in understanding Advanced Directives, i.e., Living Wills, Power of Attorney, etc.
as requested.
* Maintains a harmonious and working relationship with the Interdisciplinary team (IDT).
Attend IDT morning meetings as needed.
* Maintains confidentiality of all disclosures.
* Supports staff in dealing with their emotion in the loss of a Participant.
* Assists with faith and non faith issues in the healing process.
* Adheres to all standards, policies and procedures of St.
Paul’s Senior Homes & Services.
Adheres to the code of ethics of the Association of Professional Spiritual Counselors.
* Serves on committees and specials projects of St.
Paul’s PACE as may be requested and assigned.
