Nurse / Admissions & Discharge Planner

Position: LPN / RN Admission and Discharge Planner
Job Type: Full Time, M-F, 10am-6pm

NHC Healthcare Franklin is looking for a Social Worker/Discharge Planner to join our team! The qualified applicant for this position should hold a LPN or RN License from an accredited program.

Responsibilities include, but are not limited, to:


* Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment


* Working with individual patients and their representatives toward transition back to the community or adjustment to center life and helping navigate crisis of illness and disability


* Serving in an advocacy role for patients and their representatives in expressing and resolving grievances


* Coordinating patient's plan of care with interdisciplinary team members


* Reporting to the Director of Social Work current case management status and needs


* Maintaining confidentiality of professional information
