We have a fabulous opportunity available for you to join our awesome team as our Support Coordinator.
We support our brilliant customers who have complex mental health support needs.
We help them live independently in the community in a flexible and bespoke way to support them to meet their goals and aspirations.
The opportunity is in our community and floating support service which we call STEP (Supporting Tenancies, Enabling People) with our customers who live in a community-based setting in the Sunderland and Washington areas.
The STEP service is successfully being developed across the Northeast and is still in its infancy in our area.
This is an exciting opportunity to get involved in developing the service and supporting it to evolve.
We empower our customers to develop the independent living skills they need, to successfully move on.

Typical day as our Support Coordinator
* Creating support plans with your customers and working together as a team to ensure the smooth delivery of support.
* Supporting our customers with complex mental health needs, learning difficulties and/or dual diagnosis.
Attending MDT's, risk planning meetings etc to enable us to best support our customers.
* Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
* Carrying out risk assessments, support planning, goal setting and regular reviews.
* Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too

* We help change our customer’s lives for the better.
You’ll play your part by assisting in the delivery of tasks that are based on our customers personal support plans, their needs, and aspirations.
You’ll work closely with external agencies and professionals to assess and engage support for our customers.
* You have experience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively.
* If you’re not the biggest fan of technology, that’s not a problem to us.
You do need to know that we are quite a high-tech company and most of our application process and mandatory learning are online.
