Pay £12.61 per hour (£24,664 per annum) plus great benefits including Health Cash Plan!

Typical day as our Support Coordinator


* Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans.


* Supporting customers with independent living skills such as; budgeting, maximising customers income, cooking, engaging with external services.


* Promoting Home Group?s values within your team.


* Work in close collaboration with; local authority, NHS mental health teams, safeguarding teams and housing option teams.

You bring


* Passion to support customer to live their best life, working together with an eye for detail.


* Experience of coordinating and signposting to internal and external services.


* The ability to work on your own initiative, remain calm under pressure and have a resilient approach.


* Recognise every customer is different and you?re able to adopt a flexible, personal approach, listening to customers to understand their needs.

* Where we?re flexible with you, we do need to meet the needs of our customers.

That may mean adapting your start or finishing times on occasion.


* Working to a set rota pattern of day shifts, including weekends and Bank Holidays. 


* Able to use technology to update outcome spreadsheets, complete online learning and to collaborate with colleagues and our stakeholders within the city.


* You?ll need an Enhanced DBS check done and we pay for that.
