POSITION SUMMARY
A Licensed Practical Nurse (LPN) provides care to residents in a skilled nursing center, under the supervision of an Registered Nurse (RN), the Director of Nursing Services (DNS), and as prescribed by the physician, by following the care plan, administering medications and treatments, partnering with members of the care team to achieve positive resident care outcomes, and in compliance with clinical policies and procedures, nursing practices, and local, state, and federal regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1.
Provides nursing care to residents by following the resident care plan and under the supervision of a Registered Nurse (RN).
2.
Evaluates residents for changes of condition and reaction to medications and treatments, documents, and notifies RN, Resident Care Manager (RCM), DNS, and/or physician of changes and significant incidents.
3.
Takes temperature, pulse, blood pressure, and other vital signs and observes skin, cognitive functioning, and behaviors to detect deviations from baseline and report on the condition of residents.
4.
Performs nursing treatments and administers prescribed and PRN medication(s) in accordance with the resident care plan and as prescribed by the physician and with approved nursing techniques.
Reports resident response to nursing interventions and informs RCM of observations.
5.
Documents all care provided and observations in a timely and accurate manner including charting vital signs, medications and treatments given, writing progress notes, and completing incident reports.
6.
Transcribes and enters orders into the resident's records and seeks clarification of unclear orders from the ordering physician and/or other health care professionals.
7.
Provides coaching and general direction to care staff by making assignments, providing feedback, problem solving concerns as they arise, and communicating with DNS regarding team member performance.
8.
Treats all residents, visitors, and staff with respect, dignity, and promotes resident rights.
Displays exceptional customer service in all interactions.
9.
Assists with new admissions, transfers, and discharges.
10.
Provides comprehensive education to residents and residents' families.
11.
Promotes a positive physical and psychosocial environment for the resident that upholds residents' rights and protects residents from all forms of abuse.
