Ridgeway Post Acute is looking for a skilled and compassionate Telehealth Physical Therapist to join our team on a PRN basis.

Position: Telehealth Physical Therapist (PRN)
Location: Ridgeway Post Acute (Remote/Telehealth)
Employment Type: PRN (as needed)

Responsibilities:


* Conduct virtual physical therapy evaluations and treatments via telehealth platforms


* Develop individualized treatment plans to improve patients' mobility, function, and quality of life


* Provide guidance and exercises that can be performed at home


* Track and document patient progress using electronic health records (EHR)


* Collaborate with an interdisciplinary team to ensure continuity of care

Qualifications:


* Valid Physical Therapist license in the state of California


* Experience in telehealth or virtual care delivery is preferred


* Strong clinical skills and the ability to work independently


* Excellent communication and tech-savvy skills


* Ability to manage flexible, PRN hours
