Mountain View Post Acute is seeking a dedicated and experienced Resident Care Manager to lead and support our team in delivering exceptional care.

This role requires a current LPN or RN license and a passion for making a difference in the lives of our residents.

Your Responsibilities:


* Oversee and coordinate resident care plans to ensure quality and compliance


* Provide leadership and guidance to nursing staff


* Monitor resident outcomes and adjust care plans as necessary


* Ensure proper documentation and communication of care


* Collaborate with interdisciplinary teams to enhance resident well-being

Requirements:


* Must hold a current and active LPN or RN license


* Previous experience in a leadership or care management role preferred


* Strong organizational and communication skills
