Assistant Staff Analyst, Client Wellness

Salary: $35.23 - $50.83 Per Hour

 

SUMMARY

Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS).  HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County.  Our organization follows a hybrid work structure where employees work both remotely and from the office, as needed.  

The Assistant Staff Analyst (ASA), Wellness will report to the Associate Director of Enriched Residential Care Program (ERC) who manages the daily operations of the Wellness team which provides support to homeless individuals have been placed in 24-hour licensed care facilities and need assistance with their activities of daily living

 The Assistant Staff Analyst (ASA) is needed to support the Staff Analyst in conducting client’s services visits for individuals experiencing homelessness who require care and supervision and are stably housed.  It is expected that the ASA will assist the Staff Analyst to coordinate assessments, care planning, and services for a portfolio of clients placed in facilities within a given region.

ESSENTIAL FUNCTIONS


* Coordinate assessments, care planning, and services for a portfolio of clients placed in facilities within a given region.


* Coordinate weekly visit schedule with MCWs for the first 90 days of client move to the facility.


* Coordinate initial wellness visit within the first week of client arrival at the facility.


* Coordinate quarterly case conferencing, care plan revision, housing acuity index, and wellness visits with facility operators for TOC or Maintenance clients.


* Coordinate with public health regarding field-based services.


* Respond to incident reports and assign appropriate staff to provide intervention.


* Respond to requests for assistance from facility operators.


* Provide on-site technical assistance to facility operators related to implementing client care plans.

 


* Maintain master client roster for the assigned region and ensure that a bi-weekly census is accurately completed by each facility in the assigned region.


* Provide technical assistance to a portfolio of facilities within an assigned region to ensure high-quality care provision to all ERC clients.


* Coordinate client relocations and step downs with the RAP team.

JOB QUALIFICATIONS

Three or more years of highly complex administrative experience the health, housing, social services, or related sectors.

 

Certificates/Licenses/Clearances


* A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
