Lead Direct Support Professional

* Works cooperatively with the management team to schedule and complete medical appointments for individuals served in a timely manner.


* Completes all necessary medical follow ups and required post medical appointments in a timely manner.


* Works with medical providers and pharmacy to ensure all medications are ordered and filled as prescribed.


* Completes weekly medication reviews accurately.


* Review residents' medical files weekly.


* Assists with emergency situations, communicates and documents such incidents in accordance with Agency policies and procedures.


* Uses technology for the completion of specified job duties.


* Attends work regularly according to assigned work schedule and in accordance with Agency policy.

SECONDARY FUNCTION(S):


* Works cooperatively and effectively with stakeholders while meeting the needs of individuals served and the programs.


* Provides personal care to persons served during specified time periods.


* Available to assist Residential Managers for needed support.


* Provides transportation to the individuals served when needed.


* Works alongside Residential Manager to ensure the Emergency Food and Supplies Kit, OSHA kit, and First Aid kit is sufficiently stocked.


* Attends and participates in in-service training, staff meetings and other activities to facilitate professional development.


* Works cooperatively with others including all staff, supervisors, administrators, co-workers, individuals served, community professionals, customers, vendors and the public.


* Assumes other duties, responsibilities and special projects as needed.

EDUCATION: Associates of Arts or Science degree from an accredited college or university with emphasis in Human Services.

Bachelor's degree preferred.

EXPERIENCE: Two years' experience working with individuals with intellectual disabilities.
