Health Center Admissions and Social Services Director

Located in the heart of World Golf Village, Westminster St.

Augustine is currently accepting applications for the position of full-time Health Center Admissions and Social Services Director.

This individual will assure that the emotional and social needs of all residents are met and maintained on an individual basis in accordance with regulatory guidelines and our established standards and guidelines.

This position will also perform clinical assessments/MDS coding for potential and current Health Center residents.

Responsible for communicating with CM/planners and the admissions process as well as the coordination of the care plan.

ESSENTIAL JOB FUNCTIONS:

The Health Center Admissions and Social Services Director must comply with all applicable rules, policies, standards and guidelines related to employment within Westminster and its communities, including:

Prepare comprehensive psychosocial assessments and care plans for all residents.During the Admission process tour facility and available accommodations with potential residents and family members.

Review and obtain signatures for required documentation.
