Job Title: Social Services Director

Position Summary:

As the Social Services Director, you will play a pivotal role in enhancing the quality of life for our residents.

You will work collaboratively with residents and their families to set personalized goals, ensuring they achieve the highest level of self-care, independence, and overall well-being.

Your expertise in medically-related social services will help foster an environment that promotes physical, mental, and psychosocial wellness.

Key Responsibilities:


* Assessment and Goal Setting: Collaborate with residents and families to evaluate social and family dynamics, psychological needs, and emotional challenges.

Develop tailored care plans to address social services needs effectively.


* Emotional Support: Provide compassionate counseling and support to residents and their families, helping them navigate challenges such as physical disabilities, health-related fears, and grief.


* Healthcare Navigation: Assist residents in making informed health care decisions and facilitate access to necessary medical services, including outpatient appointments and on-site ancillary services (e.g., optometry, podiatry).


* Resource Coordination: Identify and connect residents with community and social service resources, including health and welfare agencies.

Ensure residents receive appropriate prosthetics and assistive devices.


* Family and Community Liaison: Maintain strong working relationships with interdisciplinary teams and external agencies to ensure comprehensive social services are available to meet resident needs.


* Documentation and Reporting: Accurately document resident social service status and progress, ensuring compliance with policies and procedures.


* Training and Development: Provide in-service training for staff as needed, promoting awareness and understanding of social services within the facility.


* Discharge Planning: Collaborate with appropriate agencies and individuals to facilitate smooth transitions for residents, ensuring continuity of care and necessary follow-up services.

Qualifications:


* Education: Bachelor's Degree in Social Work or Human Services; MSW preferred.


* Experience: Minimum of 2 years of supervised social work experience in a healthcare setting, with a focus on individual support.


* Skills:
+ Proficient in reading and interpreting technical documents, policies, and procedures.
+ Strong communication skills to effectively present information and respond to inquiries.
+ Ability to apply mathematical concepts to practical situations.


* Certifications: Knowledge and experience with PCC preferred.

Physical Demands:


* Frequent standing, walking, and lifting (up to 25 pounds).


* Occasional sitting and climbing, with the ability to reach, push, or pull.
