Advanced Practice Provider, Lymphatics

Nemours is seeking an Advanced Practice Provider, Interventional Radiology Programs to join our Nemours Children's Health team in Wilmington, DE.

The Interventional Radiology Programs APP will function in both an independent and expanded collaborative role, by planning and providing comprehensive management to patients with a variety of unique disorders.

The APP in this role will support patients seen through Interventional Radiology Programs, such as: Lymphatics, Vascular Anomalies and Y-90.

The patients will be seen in the inpatient and outpatient setting.

This provider must work collaboratively with an interdisciplinary team and demonstrate excellent written and verbal communication skills in all phases of practice.

The ideal candidate will be an educator and develop long-term clinical relationships with patients and their families.

Essential Functions


* Assessment, diagnosis, management, and follow up visits for patients with acute and chronic needs.


* Support patients and their families being seen through Interventional Radiology Programs - Lymphatics, Vascular Anomalies and Y-90.


* Provide concise documentation of patient encounters including diagnoses, management strategies and follow up plan.


* Identifies patient and family needs for additional resources and initiates appropriate referrals.


* Provides patient and family teaching based on identified learning needs.


* Evaluates compliance with the prescribed treatment regimen through phone calls, emails, app messaging, patient/family assessments, and outpatient visits as needed.


* Manages all cases regarding assessment and coordination - determines appropriate records, records review, requests new records and develops case presentations for case reviews.


* Acts as patient advocate and liaison for inpatient and outpatient teams.


* Acts as role model for other office members to foster professional development.


* Utilizes advanced knowledge and specialized skill in practice in accordance with the state law, hospital bylaws, and hospital policy.


* Demonstrates proficiency in the care of all age groups Including neonates, Infants, toddlers, school-aged children, adolescents, and young adults.


* Responsible for participation in departmental initiatives and programmatic development including the identification of opportunities to improve services, offer recommendations, and implement actions as appropriate and consistent with the goals of the Nemours Foundation.
