Social Worker – PEAK (Professional Evaluation and Assessment of Kids)

Work Schedule :

Full-time, 100% FTE day shift position.

Monday - Friday between the hours of 7:00am - 5:00pm, no weekends or holidays required.

Hours may vary based on the operational needs of the department.

Join the #1 hospital in Wisconsin! We are seeking a Social Worker within the new UW Health PEAK (Professional Evaluation and Assessment of Kids) program to:


* Conduct integrated biopsychosocial assessments for pediatric patients via in depth telehealth appointments.


* Clarify medical risk factors for cognitive or developmental difficulties.


* Conduct differential diagnostics for possible disorders accounting for presenting complaints (e.g., Autism Spectrum Disorder, ADHD, Specific Learning Disabilities such as dyslexia).


* Ensure the child is routed to the correct pediatric clinic for appropriate further assessment of those likely conditions.


* Provide psychoeducation for parents to request additional services and supports through the public school system.


* Identify other recommendations that may be beneficial to address the presenting symptoms.


* This social worker will be responsible for tracking parent and teacher checklists and facilitating communication across appropriate clinics.

Education:


* Minimum - Master's Degree in Social Work

Work Experience:

Preferred -


* Two (2) years prior hospital based social work experience


* Telehealth experience


* Experience with neurodiverse and/or medically complex populations


* Bilingual preferred

Certifications:

Minimum - Licensed in the state of Wisconsin with one of the following within 90 days of hire:


* Licensed Clinical Social Worker (LCSW)

Preferred - Licensed in the state of Illinois (for telehealth purposes) with one of the following within 90 days of hire:


* Licensed Clinical Social Worker (LCSW)
