Responsibilities:


* Conduct comprehensive physical therapy evaluations to assess residents' mobility, strength, and overall functional abilities.


* Develop and implement personalized treatment plans to help residents achieve their rehabilitation goals.


* Provide hands-on physical therapy treatments, including therapeutic exercises, manual therapy, and use of specialized equipment.


* Collaborate with an interdisciplinary team, including physicians, nurses, and other therapists, to ensure coordinated and effective care.


* Educate residents and their families on home exercise programs and strategies to maintain progress after discharge.


* Document treatment sessions, progress notes, and discharge plans accurately and timely.

Qualifications:


* Current California Physical Therapist license.


* Strong clinical skills with a focus on patient-centered care.


* Excellent communication and interpersonal abilities.


* Ability to work effectively both independently and as part of a team.


* Experience in a skilled nursing or rehabilitation setting is preferred, but not required.
