Outreach Specialist / Medical Assistant

Work Schedule :

Part-time, 90% FTE, day shift.

Monday - Friday between the hours of 8:00am - 4:30pm, one day off every two weeks, no weekends required.

Hours may vary based on the operational needs of the department.

This position will work remotely after training and meeting criteria to work from home.

We are seeking an Outreach Specialist / Medical Assistant (MA) to:


* Conduct annual wellness pre-visit planning.


* Chart review for Hieratical Condition Categories which are used to predict risks for patients with chronic diseases.


* Patient outreach.

Education:

Minimum - Completion of Medical Assistant program, licensure as an LPN or graduate of a school of nursing

Work Experience:

Minimum - One year experience as a Certified Medical Assistant, LPN, RN, or other clinical experience

Certifications:

Minimum - Certified as CMA, RMA or CCMA or eligible for certification.

Certification must be obtained within 1 year of hire from date of hire.

If LPN or RN, maintain state LPN or RN licensure in state where employed
