Clinical Counselor

Join Cottage Health as a Clinical Counselor.

The Clinical Counselor is responsible for facilitating group therapy, program referrals, assessments, and admissions.

Coordinates with outpatient providers and patient supports networks for safe complete discharge planning.

Major accountabilities include:


* Responsible for clinical case management to include: serving as liaison to community agencies and therapists; Communicates to medical staff and treatment team involved in patient's care; and Coordinates treatment planning sessions.


* Processes referrals, evaluations, and admissions to internal and external programs.


* In conjunction with the appropriate team members, schedules activities and leads a variety of therapeutic activity groups.

Provides individual psychotherapeutic interventions necessary to promote optimal health.

Makes required documentation in patient records.


* Assists with promoting program services to the community and serves as liaison to local agencies, programs, and clinicians.


* Assists with the development of quality assurance and improvement standards and audits charts to assure charting and procedures on all patients is complete.


* Assists with development of policies and procedures for intake, assessment, programming, treatment planning, charting, discharge planning, special treatment procedures and maintenance of a therapeutic environment in a manner compliant with The Joint Commission, HIPAA and Title 9 Regulations (DHCS).


* Participates in treatment planning and review conferences.

Provides input to the treatment planning process.

Identifies planning problems that need early intervention.

Incorporates specific discharge plans into plan of care and revises plan of care as treatment progresses.

QUALIFICATIONS

All job qualifications listed indicate the minimum level necessary to perform this job proficiently.

LEVEL OF EDUCATION

Minimum: Master's degree in psychology, counseling, and/or social work

CERTIFICATIONS, LICENSES, REGISTRATIONS

Minimum:

For Cottage Residential Center only, the following are required: 1) Valid California Driver License, AND 2) American Heart Association (AHA) Heartsaver First Aid Cardiopulmonary Resuscitation (CPR) Automated External Defibrillator (AED) OR American Heart Association (AHA) Basic Life Support (BLS), AND 3) Must be licensed/registered/certified with one of the following: Associate Marriage and Family Therapist (AMFT) (Candidate has 90 days from hire or transfer to provide proof of AMFT designation) OR Licensed Marriage and Family Therapist (LMFT) OR Registered Associate Clinical Social Worker (ACSW) (Candidate has 90 days from hire or transfer to provide proof of ACSW designation) OR Licensed Clinical Social Worker (LCSW) OR Licensed Psychologist OR Registered Psychology Assistant OR Certified Alcohol Drug Counselor or Registered intern, with an accredited Certifying Organization approved by the State of California Healthcare Services.
