Patient Access and Referral Coordinator

Liberty Resources’ Integrated Health Care is currently seeking a Patient Access and Referral Coordinator to support our growth and expansion.

Position Summary:

The Patient Access and Referral Coordinator will provide a variety of customer service functions that support the Certified Community Behavioral Health (CCBHC) Clinic Operations.

Job Responsibilities:


* Schedule and confirm patient diagnostic appointments, surgeries and specialty consultations.


* Maintain referral records to ensure that referral appointments are kept, all documents are obtained, follow up care is received and report on data.


* Performs patient referrals and authorizations by completing and submitting documents and discussing with providers, insurance carriers and patients.


* Collaborate with Patient Access Team to ensure complete and accurate registration, including patient demographic and current insurance information.


* Provide direct supervision and collaboration with Patient Access Team to ensure patient access is provided with adherence to all policies and standards as defined by Liberty Resources.


* Identifies ways to improve care of patients and make recommendations to the treatment team.


* Demonstrate the values of diversity, equity and belonging; foster an inclusive environment that facilitates diversity.


* Performs research and recommends ways to expand and improve patient access.


* Attend and participate in quality improvement initiatives, team meetings and huddles.


* Participates in cultural diversity training and team meetings.

Qualifications:


* High school diploma or equivalent.


* Medical assistant certificate or minimum of 2 years relevant experience.


* Previous scheduling and referral management experience preferred. 


* Strong organizational skills, ability to manage competing priorities, use good judgement and be resourceful in problem solving.


* Ability to take and follow through with delegated tasks and accountability.


* Knowledge of medical procedures and medical terminology.
