Work Schedule:

Monday thru Friday, 8 to 5

RESPONSIBILITIES

Provides skilled services to patients at home and manages the patient care services pertaining to the curative, rehabilitative, and/or preventive aspects of nursing pursuant to the plan of treatment when nursing is involved in a patient's care.

Work Performed:


* Makes the initial evaluation visit, when nursing is involved in the plan care and at other times, as directed by the Director of Professional Services;


* Regularly re-evaluates the patient's nursing needs;


* Initiates the nursing plan of care with appropriate preventive and rehabilitative nursing procedures and necessary revisions;


* Furnishes those services requiring substantial and specialized nursing skill;


* Prepares clinical and progress notes the day of service;


* Coordinates services;


* Informs the physician and other personnel of changes in the patient's condition and needs


* Counsels the patient and family in meeting nursing and related needs;


* Participates in in-service programs;


* Supervises and teaches other nursing personnel;


* May delegate aspects of patient care to unlicensed individuals


* Teaches and trains patients and caregivers nursing care


* Provides nail care to patients


* Performs medication management setup


* Any other duty within the scope of nursing state practice regulations.

QUALIFICATIONS



* Registered nurse with current licensure to practice professional nursing in the state.


* Graduate of National League for Nursing accredited school of nursing.


* Maintains a current CPR certification.


* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.


* Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing is...
