FNP/PMHNP - Behavioral Health Crisis & Integrated Care Company: iCare Health Services, LLCDepartment: Crisis Services / Integrated Outpatient Services/Physical HealthPosition Type: Full-Time, Part-Time, PRNSetting: Behavioral Health Crisis Stabilization, 23-Hour Observation & Integrated CareReports To: Clinical Leadership / Healthcare Administrator iCare Health Services, LLC is seeking an experienced dual-certified Family Nurse Practitioner/Psychiatric Mental Health Nurse Practitioner (FNP/PMHNP) to provide integrated medical and psychiatric care within our Behavioral Health Crisis Stabilization and 23-Hour Observation program.
The FNP/PMHNP will evaluate individuals experiencing acute psychiatric symptoms, behavioral health crises, substance-use-related concerns, and co-occurring medical conditions.
This provider will perform psychiatric and medical evaluations, establish diagnoses within applicable scope, prescribe and manage medications, order and review appropriate diagnostic testing, develop stabilization plans, and participate in decisions regarding continued observation, discharge, referral, or transfer to a higher level of care.
The provider will work collaboratively with Crisis RNs, licensed behavioral health clinicians, BHTs, case managers/care coordinators, peer support specialists, and community healthcare partners to provide safe, timely, trauma-informed, and coordinated care.
Essential Duties & Responsibilities Psychiatric and Crisis Services Conduct comprehensive and focused psychiatric evaluations.
Evaluate individuals experiencing suicidal ideation, self-harm risk, homicidal ideation, psychosis, mania, severe depression or anxiety, agitation, behavioral dysregulation, and other acute psychiatric presentations.
Establish and document psychiatric diagnoses within PMHNP scope.
Assess psychiatric stability and immediate treatment needs.
Develop individualized psychiatric stabilization and treatment recommendations.
Initiate, continue, adjust, or discontinue psychotropic medications when clinically appropriate.
Monitor therapeutic effectiveness, side effects, adverse reactions, contraindications, and medication interactions.
Participate in suicide-risk, violence-risk, safety, and crisis-management planning.
Determine when psychiatric presentation warrants continued crisis observation, inpatient psychiatric treatment, emergency intervention, or outpatient follow-up.
Provide psychiatric consultation to the multidisciplinary crisis team.
Educate patients and authorized family/support persons regarding diagnoses, medications, treatment options, safety plans, and follow-up needs.
Medical and Physical Health Services Conduct focused and comprehensive medical histories and physical examinations.
Evaluate acute physical-health concerns occurring during behavioral health crisis episodes.
Assess chronic medical conditions that may impact stabilization or psychiatric presentation.
