Job Title: Social Service Assistant full time

Vasona Creek Healthcare is seeking a compassionate and organized Social Service Assistant to join our team part-time.

You will work closely with our Social Services team to support patients, residents, and their families.

This role ensures our residents receive necessary emotional support, community resources, and discharge planning.

Key Responsibilities



* Assist residents and families with the admission and discharge planning processes.


* Identify social and emotional needs of residents and coordinate appropriate interventions.


* Maintain accurate, confidential documentation and electronic social service records.


* Coordinate with community agencies to secure external resources for residents.


* Help residents resolve grievances or concerns to ensure an optimal care experience.


* Participate in multidisciplinary care planning meetings.

Qualifications & Skills



* Education: High school diploma required; Associate's in Social Work, Psychology, or a related human services field is highly preferred.


* Experience: Previous experience in a long-term care, skilled nursing, or healthcare setting is preferred.


* Communication: Strong interpersonal skills with the ability to speak empathetically with residents and families.


* Organization: High attention to detail and ability to manage multiple administrative tasks.


* Computer Skills: Proficiency in electronic health record (EHR) systems and basic office software.
