Social Services Director (Full-Time)

Diablo Valley Post Acute is seeking a compassionate, organized, and dedicated Social Services Director to join our team.

Position: Social Services Director
Employment Type: Full-Time
Salary: $80,000-$85,000 per year

Responsibilities


* Assess residents' psychosocial needs and develop individualized care plans.


* Coordinate discharge planning and transitions of care.


* Provide emotional support, counseling, and advocacy for residents and families.


* Collaborate with the interdisciplinary team to ensure quality resident care.


* Connect residents and families with appropriate community resources and services.


* Maintain accurate documentation and ensure compliance with state and federal regulations.


* Participate in care conferences and quality improvement initiatives.

Qualifications


* Experience in social services within a skilled nursing, post-acute, or healthcare setting preferred.


* Knowledge of discharge planning, resident rights, and regulatory requirements.


* Excellent communication, organizational, and interpersonal skills.


* Ability to work collaboratively with residents, families, physicians, and healthcare professionals.
