Social Services Director

Position Summary
Mission Valley Post Acute in Fremont, California is seeking a compassionate and experienced Social Services Director to lead the facility's social services program.

The Social Services Director is responsible for addressing the psychosocial needs of residents, supporting families, and ensuring residents receive the resources and advocacy necessary to maintain the highest possible quality of life.

This role works closely with the interdisciplinary team to support care planning, discharge planning, resident rights, and regulatory compliance within a skilled nursing and post-acute care environment.

Key Responsibilities


* Develop, implement, and oversee the facility's social services program in accordance with federal, state, and local regulations.


* Conduct psychosocial assessments for residents upon admission and periodically throughout their stay.


* Participate in interdisciplinary care plan meetings and contribute to individualized care planning.


* Provide counseling, emotional support, and guidance to residents and families.


* Coordinate discharge planning to ensure safe and appropriate transitions to home or other levels of care.


* Assist residents and families with community resources, financial assistance programs, and long-term care planning.


* Advocate for resident rights and ensure residents are treated with dignity and respect.


* Investigate and document resident grievances and concerns in a timely manner.


* Work collaboratively with nursing, therapy, activities, and administration to support resident well-being.


* Coordinate family meetings when necessary to address care concerns or changes in condition.


* Assist residents with advance directives and end-of-life planning when appropriate.


* Maintain accurate documentation in compliance with regulatory requirements and facility policies.


* Ensure compliance with HIPAA, CMS regulations, and California Department of Public Health standards.


* Participate in quality assurance and performance improvement activities related to resident care and satisfaction.

Qualifications


* Bachelor's degree in Social Work, Human Services, Psychology, or related field required.


* Master's degree in Social Work (MSW) preferred.


* Previous experience in a skilled nursing facility, post-acute care setting, or healthcare environment preferred.


* Knowledge of Medicare, Medi-Cal, and long-term care regulations preferred.


* Strong communication, counseling, and problem-solving skills.


* Ability to manage sensitive situations with professionalism, empathy, and discretion.


* Experience with discharge planning and community resource coordination.


* Strong organizational skills and ability to manage multiple residents and responsibilities.
