Position Highlights:


* Provide virtual physical therapy evaluations and treatments


* Develop and implement individualized care plans


* Collaborate closely with on-site interdisciplinary team members


* Monitor patient progress and adjust treatment plans as needed


* Educate patients and caregivers on therapeutic exercises and safety

What We're Looking For:


* Active California Physical Therapist license


* Strong clinical assessment and documentation skills


* Experience in skilled nursing or post-acute care preferred


* Comfortable utilizing telehealth platforms and technology


* Compassionate, motivated, and team-oriented


* Ability to work effectively in a fast-paced environment
