Registered Nurse III MC/CHC - Enhanced Care Management - Lake Elsinore Clinic

The Riverside University Health System - Community Health Center (CHC) is seeking an experienced Registered Nurse to join our team at the Lake Elsinore Clinic.

The incumbent will be responsible for providing enhanced care-management services to diverse populations, including individuals who are homeless or experiencing substance use disorders, behavioral health conditions, or are pregnant or in the post-partum period.

Additionally, the incumbent will be responsible for c ollaborating with interdisciplinary team members to support patient care, p roviding health promotion education to patients, and pe rforming case management and care coordination for patients with complex needs.

Required Licenses & Certifications


* Current and valid license to practice as a Registered Nurse in the State of California.


* Current and valid Basic Life Support (BLS) Certification issued by the American Heart Association.

Preferred Qualifications:


* Case Management experience


* Knowledge of EPIC, Care Director, Manifest Medex and IEHP patient portal


* Bilingual in Spanish is preferred.

Work Schedule:


* 9/80, with every other Friday off


* Hours: 7:30 am to 5:00 pm

Responsible for intake and providing comprehensive case management services.

Assist physicians in administering treatments and in performing medical examinations, diagnostic tests, and surgical operations.

As directed, administer prescribed treatment and medications including dispensing as ordered to patients; chart treatment of patients and record and report significant changes in condition and general progress of patient.

Counsel and advise patients, families and significant others regarding special medical problems and/or proper healthcare methods; prepare and maintain concise and complete records and reports.
