Specialty Nurse - Solid Organ Transplant
We are seeking a Specialty Nurse for the Solid Organ Transplant team.
The Specialty Nurse is responsible for an area of specialized nursing practice and provides direct and indirect care to patients and their families in the inpatient, ambulatory, and outpatient settings.
The specialty nurse utilizes the nursing process as a basis for providing and evaluating nursing care by applying knowledge of growth, development, pathophysiology, and family dynamics.
The specialty nurse will support solid organ transplant recipients throughout the continuum from evaluation, listing, transplantation, and post-transplantation.
This will include the coordination of the evaluation of potential transplant candidates, listing of candidates, coordination of transplants, and care post-transplant.
This position will be responsible for providing ongoing education and support surrounding care of the transplant recipient to patients, families, and other health care providers and nursing staff.
This role will also include clinical duties and office coverage (messages, calls, faxes) as well as call coverage for kidney offers.
Compliance with internal and external regulatory standards including UNOS, CMS, and Joint Commission will be required.
As a member of the transplant team, the specialty nurse will need to be able to effectively work with a large, multidisciplinary care team to care for children and their families.
Position is full-time, 40 hours per week.
Responsibilities:
Practices collaboratively with physicians and other health care providers to provide high quality health care to children.
Performs appropriate assessment of patient/family upon referral/admission.
Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care.
Provides patient/family teaching based on identification of learning needs.
Participates in responding to patient phone calls and documents patient care and progress on patient's chart.
Acts as a liaison between families and members of the health care team.
Acts as patient's advocate.
Utilizes specialized body of knowledge and technical skills in nursing practice related to subspecialty in accordance with the Nurse Practice Act, hospital policy and protocol.
Participates in the development of standards of care for the specialty patient population.
Demonstrates proficiency in the care of all ages, neonates, pediatrics, adolescents, and adults.
Recognizes and identifies own learning needs and responsibility for continual development; strives to increase knowledge and identifies goals for professional growth.
Responsible for participation in departmental and hospital programs for Quality/Process Assessment and Improvement, identifying opportunities to improve services, making recommendations, and implementing actions as appropriate and consistent with the goals of the Nemours Foundation.
