What You'll Do:


* Evaluate residents and develop individualized treatment plans


* Deliver evidence-based physical therapy services to post-acute and long-term care residents


* Collaborate with an interdisciplinary team to support resident progress and outcomes


* Document services accurately and timely to ensure compliance


* Educate residents, families, and staff on mobility, safety, and home exercise programs

What We're Looking For:
✔️ Active California PT license
✔️ Strong clinical and communication skills
✔️ A team-oriented mindset and a heart for working with seniors
✔️ Experience in skilled nursing or post-acute care preferred but not required
