Job Responsibilities:


* Assess and develop individualized treatment plans for residents with speech, language, voice, cognitive, and swallowing disorders


* Implement evidence-based therapy techniques to support communication and swallowing function


* Collaborate with an interdisciplinary team, including PTs, OTs, nursing staff, and physicians, to provide holistic care


* Educate residents and families on strategies for continued progress


* Maintain accurate documentation of evaluations, treatment plans, and progress reports

Qualifications:


* Valid California SLP license


* SNF or geriatric rehabilitation experience preferred, but new graduates are welcome to apply


* Strong communication and teamwork skills


* Dedication to improving patient outcomes and quality of life
