Medication History Pharmacy Technician

The Medication History Pharmacy Technician under the supervision of a licensed pharmacist, and according to departmental policies, procedures, and guidelines, will obtain an accurate medication history by interviewing patients and consulting with their outpatient pharmacies.

MAJOR ACCOUNTABILITIES / SPECIFIC JOB COMPETENCIES

1.

Obtain medication histories on all assigned patients during scheduled shifts.

2.

Perform patient and/or caregiver interviews to obtain allergies and dose, route, frequency, indication, and date/time last dose taken of current medications including prescriptions, over the counter medications, and herbal supplements.

3.

Utilize available resources for medication history collection including, but not limited to patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records to confirm medication regimens or investigate discrepancies when necessary.

4.

Communicate medication related issues to all appropriate health care professionals.

5.

Write formal notes in the electronic medical record where appropriate, documenting the findings of the medication history.

6.

Provide exemplary customer service to CH staff and patients.

7.

Maintain the confidentiality of all designated hospital and patient information at all times.

8.

Perform quality improvement projects and other related tasks as needed/assigned.

CERTIFICATIONS, LICENSES, REGISTRATIONS


* Pharmacy Technician Licensure by the California State Board of Pharmacy


* National Pharmacy Technician Board Certification Medication History Certificate preferred

TECHNICAL REQUIREMENTS


* Computer skills and knowledge of pharmacy technology


* Familiarity with EPIC preferred

YEARS OF RELATED WORK EXPERIENCE


* 3 Years experience


* 1-2 years of related outpatient/retail pharmacy experience (order processing, prescription adjudication, third party problem resolution, and customer service preferred.
