Key Responsibilities:


* Complete and coordinate the Minimum Data Set (MDS) assessments for residents in compliance with federal and state guidelines.


* Work closely with nursing, therapy, and other interdisciplinary teams to ensure accurate and timely assessments.


* Ensure accurate documentation to support reimbursement and compliance with regulatory standards.


* Conduct care planning meetings and collaborate with staff to ensure individualized care plans.


* Maintain up-to-date knowledge of MDS regulations and Medicare/Medicaid reimbursement guidelines.

Qualifications:


* Valid LVN license in the state of California.


* Previous experience in MDS coordination in a skilled nursing or long-term care setting.


* Strong attention to detail, organizational, and communication skills.


* Knowledge of MDS 3.0 and understanding of Medicare/Medicaid regulations.


* Ability to work effectively under pressure and meet deadlines.
