The Health Coach will provide personalised support to individuals to empower them take control of their own health and wellbeing.

The post holder will facilitate people in establishing and attaining health-promoting goals set by the person in order to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life

The post holder will be responsible for delivering a range of face to face, telephonic and digital health coaching support to individuals from a wide range of settings.

Accountability

To provide largely a remote coaching service for eligible participants that supports them to gain and use the knowledge, skills and confidence to become active participants in their care and to reach their self-identified health, wellbeing and employment goals.

To deliver telephonic and digital health coaching support to a case load of participants, providing personalised psychosocial interventions including, but not limited to, agenda setting, problem solving, motivational interviewing, goal setting and signposting.

To understand the boundaries of coaching and ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately.

To organise and deliver occasional health coaching face to face support, during site based health and wellbeing events

To contribute to the monitoring and implementation of all policies and systems as they relate to service delivery, in particular: Health and Safety, Safeguarding, Vulnerable Adults

To ensure timely accurate reporting and data collection

To take a proactive approach to supervision and training with a demonstrated commitment to personal development
