Full-Time Geriatric Nurse Practitioner (Hospice/Primary/Palliative Care)

Salary: $120,000.00 to $140,000.00 DOE

Work Schedule:

Monday - Friday, 8:00AM - 5:00PM or four ten hour days, during Monday through Friday

RESPONSIBILITIES

The Nurse Practitioner will be providing acute primary/palliative care to geriatric patients in their homes; Skilled Nursing facilities, Assisted Living and Independent living facilities.

Will be providing care to 8 to 10 patients per day.

The Nurse Practitioner will practice independently and collaboratively to provide both acute and chronic care in facilities, residential care and home settings.

The Nurse Practitioner will assess, educate, diagnose, treat and refer as indicated.

The Nurse Practitioner's practice will meet evidence-based guidelines and practice service, productivity and teamwork in alignment with Signature Health care at home.



* Develop caseload based on referrals from clinical staff, community and attending physicians.


* Manage a complex caseload of patients, rounding on patients as medically necessary


* Participate in Risk and Quality Management programs, clinical meetings and other meetings as required


* Perform in-depth health risk assessments on all referred patients


* Collaborate with clinical team, including the attending physicians, to develop care plans


* Review past medical history


* Perform appropriate medical screening


* Order and interpret diagnostic results


* Diagnose and treat


* Evaluate caseload care plans and adjust treatment as indicated


* Provide prescriptions


* Participate in all mandatory compliance and other training programs


* Seek guidance for compliance related concerns and adheres to all applicable laws, regulations and company policies and procedures.


* Maintain positive relationships and rapport with coworkers, residents, family members and facility personnel.


* Manage time with the goal of providing 70-85% direct patient care.

QUALIFICATIONS



* Master's degree in nursing required, along with commensurate experience and satisfactory completion of NP licensure
