Primary hours are Monday - Friday 8:30am - 4:30pm.
We specialize in providing Home Care and Hospice in the home and facilities.

KEY JOB RESPONSIBILITIES: Provides constant and ongoing care for elderly and terminally ill patients by evaluating patient needs, creating care plans, and providing end-of-life support to patients and their families.
Works in a collaborative, team-driven environment that requires frequent communication with other Care Team members.
Additionally, the Registered Nurse will:
* RN regularly assesses and reassesses the nursing needs of the Hospice patient.
* Registered Nurse Provides dietary counseling.
* RN provides Hospice nursing services, treatments, and preventive procedures.
* Registered Nurse initiates nursing procedures appropriate for the patients Hospice care and safety.
* RN observes signs and symptoms and report to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
* Registered Nurse teaches supervises and counsels the Hospice patient and family members about providing care for the patient.
* RN supervises and trains other nursing service personnel.
* Registered Nurse develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
* RN performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
* Registered Nurse attends team conferences.
* RN follows the policies and procedures of The Care Team.
Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
* Registered Nurse always communicates to the supervisor if unable to meet a patients need or perform a procedure.
* RN participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
