Provide transportation service for the residents at the community over specified routes to local or distant points according to time schedule.
This position will be required to work 32 - 37.5 hours/week and comes with benefits.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Comply with all applicable rules, policies, standards and guidelines related to employment with Westminster Services and its communities.
1.
Inspect company vehicles for defects and safe operating condition before, during and after trips.
Complete the vehicle “Log” after each trip or at the end of the scheduled outing.
2.
Apply knowledge of commercial driving and skills in maneuvering vehicles at varying speeds in difficult situations, such as heavy traffic, inclement weather, etc.
Must be able to operate the company vehicle in all types of weather and conditions.
This includes going forward and backing up long distances, around corners, and in and around very tight areas.
3.
Report all accidents and incidents involving drivers or company vehicles.
4.
Operate the controls of the Handicap Lift – assist residents when utilizing the lift.
Secure wheelchairs utilizing the “Shure-lock” wheelchair tie down system on the bus.
5.
Assist residents on and off the vehicles and during scheduled outings.
6.
Safely operate company vehicle and follow traffic laws/regulations/local ordinances/FDOT regulations when operating the company vehicle.
7.
Assist with the preventive maintenance of the company vehicles.
ESSENTIAL QUALIFICATIONS:
Education and Experience:
* Must posses a valid CDL License with a Passenger Endorsement.
* Must posses a valid FL driver’s license free of driving violations or offences.
* Must provide copy of a valid Medical Card (CDL).
* Must have knowledge of DOT regulations governing safe driving, hours of service, inspections and maintenance.
* High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

We are currently seeking a Bilingual Care Manager to provide direct services through our Early Intervention (EI), Committee on Preschool Special Education (CPSE), our Committee on Special Education (CSE) programs and/or our private programs.
Immediate full-time or part-time positions are available throughout Hudson Valley.
Services may be provided in home-based, school-based, or center-based settings or at Liberty POST locations.

Position Summary:
Care Managers provide outreach and enrollment services to children presumed to meet eligibility requirements of New York State’s Children’s Health Home program. They gather assessments and enter documentation that confirms eligibility and identifies areas requiring supportive services. They work with children, families and service providers to create a comprehensive Plan of Health Care, identify additional service providers as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan. Care Managers document progress, transitions, and changes in eligibility.
Job Responsibilities:
• Document all case activity, including outreach, consent development and assessment, plan development, client progress and transition arrangements.
• Administer CANS-NY, updating periodically.
• Develop a comprehensive, family-driven youth-guided Plan of Health Care.
• Inventory and coordinate existing services relevant to the Plan of Care, identifying and securing additional services as appropriate.
• Convene and conduct quarterly meetings to review progress, update POCs, and confirm continuing client eligibility.
• Provide education to family and child concerning the nature and desired effect of services, along with information concerning conditions being treated.
• Assist families and children in the acquisition and maintenance of public benefits e.g., financial, educational, social, and community services.

Assist with the preventive maintenance of the company vehicles.
BUS DRIVER ESSENTIAL QUALIFICATIONS:
Education and Experience:
Must posses a valid CDL License with a Passenger Endorsement.
Must provide copy of a valid Medical Card (CDL).
Must have knowledge of DOT regulations governing safe driving, hours of service, inspections and maintenance.
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
Must posses a valid FL driver’s license free of driving violations or offences.
Must be able to lift up to 60 lbs.

The Self-Direction Associate is a professional who ensures Medicaid and NY State compliance standards are met for billing purposes in Self Direction Services.
The SD Associate will be responsible for reviewing and approving payroll for SD DSP’s (Direct Support Professional) in the eVero system.
The SD Associate will review and ensure required documents are attached to all invoices to ensure reimbursements are billable per OPWDD guidelines.
The SD Associate will ensure individual training records for self-hired DSPs are kept up to date as well as training DSP’s on the eVero app.
The SD Associate will review Service Plans, attend Circle of Support Meetings and assist with the tasks assigned by the Fiscal Intermediary Coordinator.
Duties and Responsibilities
* Well versed in all Self-Direction policies and procedures.
Training DSP’s on all SD policies as required to ensure SD policies are followed.
Using the eVero software for processing payroll, ensures EVV requirements are met when approving payroll.
Making Fiscal Intermediary Coordinator (FI) aware of DSP’s that require a disciplinary memo per SD policies.
* Correspond with self-hired DSPs to communicate training needs to stay up to date with compliance standards.
Monitors Relias training records and employee files on SharePoint.

Under the direct supervision of the Children’s Care Management Supervisor, the Children’s Health Home Care Manager (CHHCM) will assist youth who are eligible for and in need of care coordination by identifying and addressing physical, behavioral and social health needs.
