General Purpose
The Staff Physical Therapist evaluates and treats patients, communicates with families, physicians and other health team members and maintains documentation of services in the medical records.
Conducts in-services and training for facility staff on an ongoing basis.
Essential Duties
• Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral.
If unable to complete evaluation in 24 hours, documentation in medical record clearly defines cause for delay.
• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Supervise Physical Therapy Assistants in direct patient care and patient related activities, following state practice act.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.
• Participate in in-services training program for other staff in the facility.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per PT Board State Practice Act and governmental and third party payer requirements.

• Ensure that the Nursing Service Procedures Manual is current and reflects the day-to-day nursing procedures performed in this facility.
• Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
• Make written and oral reports/recommendations to the Director as necessary/required, concerning the operation of the nursing service department.

General Purpose
The primary purpose of this position is to provide routine daily nursing care and services to assigned residents in accordance with each resident's assessment and care plan, and as directed by nursing supervisors.
Essential Duties & Responsibilities
Resident Care
* Identify residents using wristbands or photo ID prior to care, meals, or treatments
* Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and oral care
* Measure and record vital signs (TPRs), weights, and intake/output as instructed
* Assist with lifting, turning, positioning, and transferring residents using proper techniques and equipment
* Keep residents clean, dry, and comfortable at all times
* Observe and report changes in resident condition, including skin breakdown, injuries, or behavioral changes
