Case Manager Position Summary:
The Case Manager performs case management-related functions to coordinate, facilitate and maintain care for the residents of Venture House and the Apartment Treatment program in Madison County.
Case Manager Job Responsibilities:
* Completes assessment of housing and housing related needs
* Works with Venture House and Apartment Treatment staff, and multi-disciplinary treatment team to establish individualized service plans, which meet the clients’ goals.
* Establishes and maintains effective relationships with other service providers: e.g.
Madison County DSS, SPOA Coordinator, PROS, Care Coordination, etc.
* Acts as program liaison and primary contact for other service providers as it relates to clients coordination of services.
* Demonstrates current understanding of the Office of Mental Health regulations, mental health disorders and its treatments. Understands reciprocal relationships between substance abuse and mental health.
* Assists individuals with linkage and referral to substance use disorder and mental health services where necessary.
* Responds to and effectively handles emergencies, which may require flexibility of time and area of assignment.
* Completes all required documentation, in timelines in accordance with program standards.
Case Manager Qualifications:
Bachelors/Associates and one (1) year experience or HS Diploma and three (3) years related experience. Must have a valid driver's license and access to reliable transportation.
Pay Rate: $21.02/hour
